Support Coordination (SC)
Support Coordination (SC) is a service that uses a Person-Centered approach to assist individuals with Developmental and Intellectual disabilities in gaining access to needed program and state plan services, as well as needed medical, social, educational and other services. The service is offered by State of New Jersey, Division of Developmental Disabilities (DDD) for adults 21 and over with Intellectual/Developmental Disabilities. Support Coordination is managed by a Support Coordinator..
Community Based Supports (CBS)
Through Community Based Supports, an Individual can receive direct support and assistance from Direct Support Professionals in or out of their home, with or without their caregiver present, to achieve and maintain the outcomes of increased independence, productivity, enhanced family functioning, and inclusion in the community.
Respite (R)
Respite is a service that is provided to Individuals who cannot care for themselves and whose main caregiver is absent or in need of momentary relief of care giving duties. Our Respite service is provided in or out the Individual’s home depending on the need of Individual and family. Our Respite service provides temporary relief from the demands of caregiving, which helps reduce overall family stress. This often enables families to better meet the needs of their loved one.
Community Inclusion (CI)
At Mercy Drive Regional Inc. we provide Community Inclusion services to Individual’s outside of their home, that supports and assist Individuals in educational, enrichment or recreational activities as outlined in his/her Service Plan that are intended to enhance inclusion in the community. Community Inclusion Services are delivered in a group setting not exceeding six individuals in a group.
Cognitive Rehabilitation
Cognitive Rehabilitation is a systematic, functionally oriented service of therapeutic cognitive activities. Services are directed to achieve functional changes by reinforcing, strengthening or re-establishing previously learned patterns of behavior or establishing new patterns of cognitive activity or compensatory mechanisms for impaired neurological systems.
Behavioral Supports
Behavioral Supports includes a complete assessment of the challenging behavior(s), development of a structured behavioral modification plan, implementation of the plan, ongoing training and supervision of caregivers and behavioral aides, and periodic reassessment of the plan. Intervention modalities are developed identifying challenging behavioral need of the individual.
Prevocational Training
Our Community Based Pre-Vocational program prepares individual for paid employment or unpaid meaningful community activities, including volunteering, in integrated community settings. In order to deliver the best job readiness training, we assess the individual in order to develop potential community and employment experiences in an integrated setting appropriate to the needs of each person. We work with each individual on their abilities to get them ready for the world of work and all its nuances and challenges. We focus on “soft skills” training, which are skills that can be utilized in many different settings. Working in a variety of employment and/or volunteer locations in the community allows the individual to explore the many different types of jobs and find what they are interested in. We assist individuals and families on benefits management pertaining to their employment.
