Support Coordination (SC) is a service that uses a Person-Centered approach to assist individuals with Developmental and Intellectual disabilities in gaining access to needed program and state plan services, as well as needed medical, social, educational and other services. The service is offered by State of New Jersey, Division of Developmental Disabilities (DDD) for adults 21 and over with Intellectual/Developmental Disabilities. Support Coordination is managed by a Support Coordinator.
Support Coordinator manages Support Coordination services for each individual by performing the following four general functions: Individual discovery, Plan development, coordination of services, and monitoring.
Support Coordinator assist the Individual to determine his/her outcomes, supports and service needs. The Support Coordinator develops and maintains the Individualized Service Plan (ISP) with the Individual, their family, and other team members designated by the Individual. The Support coordinator assists Individual/families to obtain the supports and services in their own communities.
- We have Support Coordinators who are very qualified to provide services to Individual and family in several languages taking in consideration the cultural diversity of New Jersey. We have bilingual support coordinators who can deliver the service in Spanish, Russian, French Creole, African Dialects, Punjabi, Bengali, Hindi, Urdu, Nepali languages.
- We have received authorization from New Jersey Department of Human Services, Division of Developmental Disabilities to approve Individualized Service Plan (ISP) for Individuals that we service.
- We complete ISP and approve it by 30 Days from the date the Individual is enrolled in to Support Coordination. We have emergency coordinators who are available 24 Hours / 7 Days a week to assist Individual/families in case of a situation of emergency.
